My Qualifications
Be The Change Home Care is committed to providing a comprehensive range of support services to enhance the physical, emotional, and mental well-being of our members. Our goal is to empower each member to actively participate in their community, making choices that align with their preferences and interests. We strive to foster self-help, socialization, adaptive skills, and positive relationships, while encouraging interaction and integration within the community.
My Approach
The health and safety of our members are of utmost importance to us. All support services will be tailored to meet the specific goals outlined in the Individualized Service Plan (ISP) of each member. To ensure that each member receives the best possible care, a specific teaching strategy will be implemented for each habilitative outcome within 20 days of initiating services or whenever a new outcome is identified.
My Specialties
To maintain transparency and accountability, we will provide the member's representative with monthly updates on all healthcare appointments. Additionally, a detailed log of the member's personal belongings will be continually updated and readily available upon request.
Our financial practices will be transparent as well. We will maintain a ledger and documentation of all member funds expenditures, submitting a monthly accounting of expenditures to the member's representative payee.
Transportation support will be provided to facilitate participation in program activities and community engagement.
Attendance records of the members will be accurately maintained on file, and a daily record of hours worked by each direct service staff will be available for reference.
